hazards that were
causing or likely to cause death or serious physical harm to employees in
that an employee was
exposed to a fall hazard of approximately ten feet:
(a)On or about January 2008, for an employee cleaning the snow off
the tops of trucks with a rake from the top of the baler in the yard
that was equipped with a railing system that was only 32" in height
and did not completely enclosed the open sides to prevent falls.
Feasible and useful abatement methods for reducing this hazard,
include but are not limited to the following:  erecting a guardrail
system around all the open sides of the baler with a top railing at
42" high and a mid-rail at 21" high.
