were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of explosion and fire:
a) Location - Establishment:
On or about November 22, 2006, the employer did not develop and implement
a safety
and health program to minimize the risks of explosion and fire for
employees
occasionally working with large quantities of flammable liquids in
production or
processes.
Among other methods, feasible means of abatement would be to institute a
process safety
management system described in "Guidelines for Technical Management of
Chemical
Process Safety," American Institute of Chemical Engineers. The system
would include:
1. Conduct, implement, and document a process hazard analysis to identify,
evaluate,
and minimize risk associated with those hazards. Methods could include use
of What-if
scenarios and Fault Tree Analysis.
2. Develop, implement, and document a system to ensure that equipment,
systems,
structures, and components are maintained in a fit-for-duty manner that
does not increase
risk of a process.
3. Develop and implement a system to acquire, evaluate, disseminate, and
maintain an
archive of regulations, standards, and codes applicable to the company.
