On or about 12/20/06, the maintenance employee was exposed to the hazard
of falling
off the fiberglass stepladder or losing his balance while on the
fiberglass stepladder while
climbing all the way up it and back down, using it to access the fixed
metal ladder.
Among other methods, one feasible and acceptable method to correct this
hazard is to use
a type of ladder that is approved for this purpose, as specified by ANSI
A14.5-2000,
American National Standard for Ladders - Portable Reinforced Plastic -
Safety
Requirements, Section 9.3.1.
(b) Location - Fiberglass ladder next to fixed metal ladder:
On or about 12/20/06, the required marking on the first step below the top
cap stating
"Danger - Do not stand on or above this rung - You can lose your balance"
was illegible.
Among other methods, one feasible and acceptable method to correct this
hazard is to put
on a new "Marking No. 3" on the first step below the top cap, as specified
by ANSI
