Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing hazards:
(a) Location - Test area:
On or about 12/28/06, employees were exposed to crushing hazards while
working with
overhead, underhung cranes that were not tested and inspected in
accordance with ANSI
B30.11-1980.
Among other methods, one feasible and acceptable means to correct this
hazard would
be to have qualified individuals test and inspect the cranes in accordance
with ANSI B-
30.11-1980.  Conduct an acceptance load test of not more than 125of the
rated load
upon initial use.
Daily or before each use (daily to monthly intervals):
All functional operating mechanisms for maladjustment interfering with
proper operation.
Deterioration or leakage in lines, tanks, valves, drain pumps, and other
parts of air or
hydraulic systems. Rope reeving for noncompliance with manufacturer's
recommendations.  Hoist chains, including end connections, for excessive
wear, twist,
distorted links interfering with proper function, or stretch beyond
manufacturer's
recommendations. Visually inspect daily; monthly inspection with a
certification record
which includes the date of inspection, the signature of the person who
performed the
inspection and an identifier of the chain which was inspected.
Hooks with deformation or cracks. Visually inspect daily; monthly
inspection with a
certification record which includes the date of inspection, the signature
of the person who
performed the inspection and the serial number, or other identifier, of
the hook
inspected.
Discard hooks with cracks or having more than 15 percent in excess of
normal throat
opening or more than 10 percent twist from the plane of the unbent hook
Periodic: (1-12 month intervals):
Have qualified individuals inspect and document:  findings of deformed,
cracked, or
corroded members.  Inspect for:  Loose bolts or rivets, cracked or worn
sheaves and
drums.  Worn, cracked or distorted parts such as pins, bearings, shafts,
gears, rollers,
locking and clamping devices.  Excessive wear on brake system parts,
linings, pawls,
and ratchets.  Load, wind, and other indicators over their full range, for
any significant
inaccuracies.  Gasoline, diesel, electric, or other powerplants for
improper performance
or noncompliance with applicable safety requirements.  Excessive wear of
chain drive
sprockets and excessive chain stretch.  Electrical apparatus, for signs of
pitting or any
deterioration of controller contactors, limit switches and pushbutton
stations.
