Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fall hazard:
Employees were exposed to a fall hazard of approximately thirty-five (35)
feet to
the bottom of the hopper floor due to improper placement of the Crosby
Clip(s)
at the anchorage point on a spliced wire rope horizontal lifeline with an
improper
eye termination.
This occurred on or about January 5, 2007 at the new fly ash hopper(s) of
the
International Power-Coleto Creek plant, 45 FM 2987, Fannin, Texas 77960.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be installation of a properly installed one-half (1/2)
inch,
continuous wire rope lifeline with no splices using properly placed crosby
clips
with the saddles placed on the live side of the one-half (1/2) inch wire
rope
horizontal lifeline, along with the use of a properly formed eye
termination which
is formed by incorporation a properly sized thimble, as required by the
manufacturers recommended specifications and as specified in ANSI Z359.1,
1992.
