Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which was free from recognized
hazards which were
likely to cause death or serious physical harm to employees in that,
employees were exposed to
the hazards of being crushed:
a) On or about February 6, 2007 and at times prior, employees were exposed
to injuries in that,
the wire rope cable on the two hoists had burrs.
b) on or about February 6, 2007 and at times prior, employees were exposed
to injury in that,
Tons Log records were not being kept during drilling operations.
Among other methods, one feasible and acceptable method to correct this
hazard is for the
employer to comply with American Petroleum Institute Recommended Practice
9B Sections
9.6.6.

employment and a place of employment which was free from recognized
hazards which were
likely to cause death or serious physical harm to employees in that, the
mast
erection and
support pole was bent:
a) On or about February 6, 2007 and at times prior, employees were exposed
to injuries in that,
the hydraulic mast was improperly repaired.
Among other methods, one feasible and acceptable method to correct this
hazard would be to
ensure all repairs are made in accordance with the American Petroleum
Institute Recommended
Practice 4G Section 6.2.4.
