Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing injuries while operating equipment that did not
have an adequate
preventive maintenance program:
a)  On or about February 28, 2007 and at times prior, Ricway Roustabout &
Water
Service did not ensure that employees were protected from truck operation
hazards.  The
water trucks were not inspected and maintained in a proper fashion.
1.  The company did not require drivers to do a complete daily inspections
and
record the results.
2.  Trucks were not given regular operational inspections of safety areas
such as
brakes, suspension, tires, and steering by a qualified inspector/mechanic.
Among other methods, one feasible and acceptable method to correct this
hazard is to create a
preventative maintenance program which would include daily and timed
maintenance inspections
of all trucks that comply with the manufactures recommendations and Title
49 CFR Part 396.
