Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to potential failure of the personal fall arrest
system from an
anchorage which was not certified:
(a) On or about May 23, 2007 and at times prior, employees were exposed to
a fall
hazard in excess of four feet to the ground below in that, the anchorage
point of the fall
protection system had not been certified or load tested for 5000 pounds of
force. In the plant
one employee had been using the A frame as a fall arrest system. The
employee was
connected to the mobile A frame while working on the concrete pipe
performing patching
operations.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard
is to:
(a)  Follow the requirements listed in ANZI Z359.1-1992 (r1999) Safety
requirements for
Personal Fall Arrest Systems.
