Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to fire and explosion hazards: a) On or about June 16, 2007 and at time prior, CDX Resources LLC, Rig 10, employees were exposed to injury in that, the employer did not ensure that no smoking rules were enforced. Among other methods, one feasible and acceptable abatement method to correct this hazardous condition would be to train all employees on the requirement of RP 54, Section 7.1.3, and ensure the requirements are followed. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").

Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, the toolpusher's trailer was not grounded: a) On or about June 16, 2007 and at time prior, CDX Resources LLC, Rig 10, employees were exposed to injury in that, the toolpusher's trailer was not grounded. Among other methods, one feasible and acceptable method to correct this hazard is to ensue all equipment is properly grounded in accordance with RP 54, Section 6.1.16. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").
