Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, employees were exposed to an explosion hazard: a) On or about October 29, 2007 employees were exposed to injury in that, a guy line had been anchored to a well head. Among other methods, a feasible and acceptable method to correct this hazard would be to ensure the rigs guy lines are properly connected to an adequate anchor that meets the specifications of the manufacture's recommendations and the API Recommended Practices 4G appendix E. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm from being struck by in that, the air hoist wire rope was kinked: a) On or about October 29, 2007 and at times prior, employees were exposed to injury while operating the two air hoists which had kinked the wire ropes. Among other methods, one feasible and acceptable method to correct this hazard would be to establish a wire rope inspection program in accordance with the manufacture's recommendations. Ensure all wire ropes that fail inspection are either taken out of service or replaced. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").
