Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm in that
employees were exposed
to fire and explosion hazards.
Establishment:  On or about 8/10/07, an employee opened liquefied
petroleum gas cylinders to
feed a heating torch and escaping gas ignited and burned the employee.
Feasible and acceptable abatement methods include:
1)  Use quarter-turn valves on fuel supply cylinders that indicate the
position of the valve in the
on or off position.
2)  Use flame detecting controls to stop the flow of fuel gas in the
absence of a flame.
3)  Develop and implement standard operating procedures and ensure the
employees follow
correct procedures when using fuel gas and heating torch equipment.
4)  Train employees in the hazards, standard operating procedures, and
proper use of fuel gas
and heating torch equipment.
