General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to the hazard of being crushed by unstable vertical bundles of plywood, while unloading plywood from freight cars. On or about 10/16/08 an employee was fatally injured when a bundle of plywood fell on him. Among other methods, one feasible and acceptable abatement method to correct this hazard is to establish written procedures and provide training to employees requiring the employees to remain on the forklift when entering freight cars to unload the car.
