A) LOCATION: 752 TURNPIKE ROAD, SHREWSBURY MA
TWO WORKERS WHERE OBSERVED EXITING A 14 FT DEEP TRENCH IN WHICH A
LADDER WAS USED AS A MEANS OF EXIT. THE TOP OF THE LADDER WAS
LOCATED WITHIN THE SWING RADIUS OF THE EXCAVATOR AND EMPLOYEES
WERE OBSERVED CLIMBING UP THE LADDER WHILE THE MACHINE WAS BEING
ROTATED.
