MDI High School - On or about January 22, 2007, an employee was injured
when he
became entangled within an improperly stored cable and hook assembly.
Among other methods, one feasible and acceptable means of abatement would
be to follow the
operator's manual and ensure individuals are clear of any moving parts,
mechanisms or
components of the unit before actuating the controls, store hook properly
on tailgate, equip and
operate lifting hooks with safety latches, instruct all employees in the
manufacturer's
instructions, equip trucks with wheel chocks, and ensure the operator's
manual is readily
available.
