approximately 50-feet above ground level was exposed to hazards in the
following instances:
a)  The EZ Stop II Shockwave Double Lanyard with Energy Absorber used by
the employee was
not used properly in that the carabiners were connected directly to its
own webbing.
Feasible and acceptable abatement methods would include:
Ensure all employees are trained in the use of all personal protective
equipment they are required
to use.
