Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to a fall hazard:
On or about 2/25/09 at 4338-54 Terrace St, Philadelphia,PA, employees were
exposed to a fall hazard
from approximately 13 feet above ground level while unloading  windows
from a Pettibone Rough
Terrain Telescoping Boom Forklift, Model 8044, that did not have
restraining means such as
guardrails on open sides.  A feasible means of abatement is to  comply
with the Manufacturer's
recommendation of not using platforms on the rough terrain forklift to
carry, support raise or lower
personnel.  Place the guards on the platform and position the load in a
manner that can be unloaded
without requiring the employees to walk on the forklift platform.
NOTE:  ABATEMENT DOCUMENTATION IS REQUIRED.
