Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards from fallen loads: (a) Location - Fabrication shop: On or about 2/13/07, employees were exposed to crushing and struck-by hazards from a 1/2-ton Yale underhung hoist that did not have proper identification on the pendant controls. Among other methods, one feasible and acceptable means to correct this hazard would be to properly identify the pendant function controls in accordance with the most applicable version of ANSI/ASME B30.11.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuires from fallen loads: (a) Location - Fabrication shop: On or about 7/27/07, employees were exposed to crushing injuries from a Detroit 5-ton underhung crane that did not have a working upper limit switch. Among other methods, one feasible and acceptable means to correct this hazard would be to ensure that the upper limit switch is functioning properly in accordance with the most applicable version of ANSI/ASME B30.11.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: (a) Location - Throughout facility: On or about 7/27/07, employees were exposed to crushing hazards from walking in the fall zone while assisting in the transportation and delivery of granite slabs. Among other methods, one feasible and acceptable means to correct this hazard is to properly train workers on the safe handling of granite slabs. This includes but is not limited to: developing clear work practices for all involved with stone handling and transportation; prohibiting the use of hand guiding stone slabs as they are moved with the fork truck; attaching a tag line to maintain safe distances while guiding the slabs in transit.
