were exposed to THE HAZARD OF BEING STRUCK-BY or CRUSHED-BY MOVING
ELEVATORS:
a)On or about 7/11/07, Employees were performing emergency evacuation of
passengers on the elevators.
Among other methods of abatement, feasible and acceptable means of
abatement is to:
1) Provide written emergency evacuation procedures for the hydraulic
elevators in their facility.
2) Train all personal involved in emergency evacuation activities involving
hydraulic elevators on the procedures, and operation of all elevator
functions.
3) Provide proper tools to access the elevators, such as an elevator door
key
that would work.
3) Follow the American Society of Mechanical Engineers (ASME) A17.1 -
2000 Safety Code for Elevators and Escalators.  Section 8.6.10.4 and ASME
A17.4 - 1991 Guide for Emergency Evacuation of Passengers From Elevators.
