recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to THE HAZARD OF BEING STRUCK-BY:
a)Prepress area, on or about 02/12/2008, a Coffing 2-ton overhead electric
hoist
had a upper limit safety switch that did not function in accordance with
the
manufacturer's manual. The monorail was not load rated.
b)Press 40-inch, on or about 05/31/07, a Coffing 1-ton overhead electric
hoist
had a safety latch on the hoist hook that had been removed. The monorail
was not load rated.
c)Press # 40, on or about 02/12/2008, a CM Lodestar 1-ton overhead electric
hoist had a upper limit safety switch that did not function in accordance
with
the manufacturer's manual. The monorail was not load rated.
Among other methods, several feasible and acceptable abatement methods to
correct
this hazard are to:
1.Repair or replace the upper limit switches and ensure they are adjusted
to
meet the requirement of the manufacturer's operation manual, Section:
Checking limit switch operation and ANSI B30.16.-1993, Overhead Hoists,
Section 16.2.3.3 Adjustments, Repairs, and Replacements.
2.Ensure that the load capacity is legibly marked on the hoist or the block
along with warnings to inform the operator of cautionary requirements
during operation. ANSI B30.16-1993, Overhead Hoists, Sections 16-1.1,
Marking and Section 16-1.1.4 Warnings.
3.Conduct frequent and periodic inspections of the hoists in accordance
with
ANSI B30.16-1993 Overhead Hoists, Section 16-2.1 Inspection.
d)Prepress area, on or about 02/12/2008, the Overhead hoist monorail
supporting a Coffing 2-ton electrical hoist did not have a rated load
capacity
marked on the rail.
e)Press #40, Hoist #9, on or about 02/12/2008, the Overhead hoist monorail
supporting a Coffing 1-ton electrical hoist did not have a rated load
capacity
marked on the rail.
f)Press #40, Hoist #10, on or about 02/12/2008, the Overhead hoist monorail
supporting a Coffing 1-ton electrical hoist did not have a rated load
capacity
marked on the rail.
Among other methods, several feasible and acceptable abatement methods to
correct
these hazards are to:
1.Mark the rated load capacity on the rail ANSI B30.11-1980, Section
11-2.1.2
Rated Load Markings-Monorails.
2.Conduct frequent and periodic inspections of hoist monorail systems in
accordance with ANSI B30.11-1973 Monorail Systems and Underhung
Cranes, Section 11-2 Inspection, Testing, and Maintenance.
Abatement certification must be submitted for these items.
