were exposed to THE HAZARD OF BEING STRUCK-BY:
a)East Assembly line Side panels, on or about 04/02/2008: A side panel
lifting
device approximately 2 1/2 feet log with chains and hooks welded to each
end
had been locally designed and fabricated as a below-the-hook lifting device
and the device had never been initially tested and rated, no rated load
markings and no inspections or external code mark record(s) was used to
determine external conditions.
b)East Assembly line Roof Units, on or about 04/02/2008: An locally
manufactured extendable below-the-hook lifting device, designed to lift
roof
assemblies, had not been initially tested, had no rated load markings and
no
inspections or external code mark record(s) was used to determine external
conditions.
Among other methods, feasible and acceptable methods to correct each of
these
hazards are to:
1.Initially test, rate, and mark the load capacity of the lifting devices
on a
tag/plate or equivalent. Section 20-1, ANSI/ASME B30.20-1993, Below-the-
Hook Lifting Devices.
2.Conduct frequent and periodic inspections of the lifting devices in
accordance
with Section 20-1, ANSI/ASME B30.20-1993, Below-the-Hook Lifting Devices.
c)East Assembly line side Panels, on or about 04/02/2008, a Budgit 1/2 ton
overhead electric hoist had an upper limit safety switch that did not
function
in accordance with the manufacturer's manual.
Among other methods, several feasible and acceptable abatement methods to
correct
this hazard are to:
1.Repair, replace or adjust the upper limit switch and ensure it is
adjusted to
meet the requirement of the manufacturer's operation manual, Section:
Checking limit switch operation and ANSI B30.16.-1973-1993, Overhead
Hoists, Section 16.2.3.3 Adjustments, Repairs, and Replacements.
2.Conduct frequent inspections of the hoists in accordance with ANSI
B30.16-
1993 Overhead Hoists, Section 16-2.1 Inspection. Paragraph 16-2.1.2(c)(2)
requires an upper limit switch test at the beginning of each shift.
Abatement certification must be submitted for items a & b only.
