40.1-51.1(a) of the Code of Virginia:  The employer, Albemarle County
Transportation Facility, did not
furnish a place of employment which was free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that, employees were
operating a Wright Hand Chain Powered
Hoist, located in the Vehicle Maintenance Shop, which exposed them to:
(a) Contusions by not having the load rating posted on the supporting
structure of the hoist.  One feasible and
acceptable method, among other methods, to correct this hazard would be to
post the load rating of the
supporting structure which would be visible from the working area such as
that called for by ANSI B30.16-1987, American National Standard Safety
Code for Hoists.
(b) Contusions by not conducting periodic (annual) documented inspections
of loose bolts, excessive wear of
chain, chain stretch, hook, end connections of load chains and the
supporting structure.  One feasible and
acceptable method, among other methods, to correct this hazard would be to
conduct annual inspections such as
that called for by ANSI B30.16-1987, American National Standard Safety
Code for Hoists.
