40.1-51.1(a) of the Code of Virginia:  The employer, Stowe Woodward LLC,
did not furnish a place of
employment which was free from recognized hazards that were causing or
likely to cause death or serious
physical harm to employees in that employees were operating a 40K Pound
Crane Pro Overhead Crane where
the hooks at the end of slings had latches that would not close the throat
opening.  These hooks were not
removed from service or moused until the latch was replaced or repaired.
In the Production Area, two hooks used to lift product weighing
approximately 3000 pounds had been provided
with latches that did not spring back into place after loading product on
the hook.  Employees could suffer
injury if the product became inadvertently unhooked.
One feasible and acceptable abatement method, among other methods, to
correct this hazard would be to repair
or replace the hook latches such as that called for by ANSI B30.10c-1992,
American National Standard Safety
Code for Hooks.
