Section 5(a)1 of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to struck-by hazards:
Employees working drilling rig #1 at Bergkamp Well# 1-8, SEC 8, TWP 23, R
32 in
Garden City, Kansas were exposed to struck-by hazards created by the
whipping action
of high pressure mud hoses in the event of a failure at the discharge from
the mud pumps
and the connection to the piping at the base of the drilling platform.
Among other methods, one feasible and acceptable method to correct this
hazard is to secure the
hose ends using cables or chains properly located and of sufficient
strength to resist hose
whipping should it become disconnected or broken and/or replace damaged
equipment as
referenced in paragraph 9.13.4 of API RP 54, Recommended Practice for
Occupational
safety
for Oil and Gas Well Drilling and Servicing Operations, 1999 edition.
29 CFR 1903.19(d)(1) requires certification and documentation that the
abatement of the
above violation is completed.
