Section 5(a)1 of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to electrical shock, struck by and fall hazards:
Employees working from elevated scissor lifts in the fabrication area of
the facility were exposed
to electrical shock, struck-by and fall hazards while replacing and or
preforming maintenance
on the facility lighting system. Procedures had not been developed or
implemented to protect
employees while working from an elevated lift such as, but not limited to:
locking out overhead
crane, placing warning beacon on scissors lift, or coordinating
maintenance
activities with
production. Employees were exposed to coming into contact with unguarded
energized electrical
conductors or struck by crane movement, creating approximately twenty-foot
fall hazards.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS: Establish local procedure, inform and train
employees on
the special precautions to be taken whenever working in the same area as
moving
equipment per ANSI standard 92.6-1990, section 8.10.2 "Warnings such as,
but not limited
locking out the crane, flags or roped off areas, flashing lights, and
barricades shall be
used".
29 CFR 1903.19(c)(1) requires certification that the abatement of the
above violation is
completed.
