Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees employment and a place of employment
which were free
from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to caught-in hazards:
In the area with the Plasma cutting table, an employee was exposed to the
hazard of being
caught in the point of operation on the big green 150 ton press brake
being used as a punch
press, due to tha fact that the foot pedal used to initiate the stroke of
the machine was not
guarded to prevent unintended actuation.
Among other methods, one feasible and acceptable method of abatement is to
install a foot
control cover to prevent unintended actuation of the press brake in
accordance with ANSI
B11.3-2002, American National Standard for Machine Tools - Safety
Requirements for
Power Press Brakes which states:
When provided, foot controls shall be protected to prevent actuation from
falling
objects, or from unintended actuation.

serious physical harm
to employees in that employees were exposed to struck-by hazards:
Employees were lifting steel sheets that weighed in excess of the rated
capacity of the P&H
Zip Lift hoist to place them in the plasma cutter.  The sheets weighed
between approximately
1,020 to 1250 pounds and the hoist had a rated capacity of 1,000 pounds.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard
is to ensure the material lifted is not in excess of the rated load
appearing on the hoist or
load block in accordance with American Society of Mechanical Engineers
(ASME) B30.16-
2003 Overhead Hoists (Underhung) Section 16-3.1.2 Applying the Load.

exposed to the
hazard of being struck-by the load suspended from the P&H Zip Lift hoist
and the employer
failed to establish and follow a program of frequent and periodic
inspections
of the hoist
system.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to establish and follow a program of frequent and periodic inspection
of the hoist system in
accordance with American Society of Mechanical Engineers (ASME)
B30.16-2003 Overhead
Hoists (Underhung) Section 16-2.1.2 Frequent Inspection and 16-2.1.3
Periodic Inspection.

to employees in that employees were exposed to struck-by hazards:
Employees loading 5/8-inch thick steel sheets onto the plasma table, were
using below-the-
hook lifting devices that were manufactured at the facility.  The lifting
devices, an S-hook
and a V-hook, were not proof tested, inspected or marked for their rated
capacity prior to
being put into service.  The parts lifted with the devices ranged in weigh
from approximately
1,020 pounds to 1,200 pounds.
Among other feasible method, one feasible and acceptable method to correct
this hazard is to
ensure the below-the-hook lifting devices are proof tested, inspected and
marked
in
accordance with ASME B30.20 - 2006 American Society of Mechanical
Engineers, Below-
the-Hook Lifting Devices, Section 20-1.2.1 Marking, Section 20-1.3.1
Inspection, and
Section 20-1.3.8.2 Load Test.
