Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees working to clean the building rafters were exposed to falling approximately 16 feet 5 inches from a platform attached to a Clark Lift Truck (model # NP 300D) and the work platform was not secured to the lifting carriage or forks. Among other feasible and acceptable methods to correct this hazard is to provide a platform that meets the requirements stated in ASME B56.1 - 2004, paragraph 7.37 (b) ***Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c)***
