Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to fire and explosion hazards:
(a)Mirror and Etching - Employees used a Cincinnati buffer, type
204-202-8, model SPL, serial
number 250327; a Baldor Buffer, number 332B, serial number P3-96; a Dieter
Kupferschmid
sander/buffer, model number 090002, employer machine number 906; a floor
mounted
pedestal buffer, employer machine number 907; and a semi-automatic mirror
buffer, employer
machine number 430 to buff and polish stainless steel and brass dental
hand instruments.  The
employer had this equipment connected to a dust collection system.
Ductwork, with numerous
90 degree bends and long runs, connected the dust-producing machinery to a
Sternvent dry-
type cyclone dust collector.  The employer had the Sternvent dry-type
cyclone dust collector
located inside the building, with no barriers located between the dust
collector and personnel,
and with no deflagration venting.  The employer did not have the buffing
and polishing
equipment, ductwork, and dust collector bonded and grounded to dissipate
the accumulation of
static charges.  Ductwork, consisting of PVC pipe, a non-conductive
material, connected the
Cincinnati buffer, type 204-202-8, model SPL, serial number 250327, to the
dust collector.
Also, the employer did not have the dust collector emptied on a daily
basis, on or about March
22, 2007.
(b)Mirror and Etching - The employer did not have the power supplies of
the buffing and
polishing equipment interlocked with the airflow from the exhaust fan on
the Sternvent dry-
type cyclone dust collector so that improper functioning of the dust
collection system would
shut down the buffing and polishing equipment.  Also, the buffing and
polishing equipment did
not have time delay switches to prevent the motor drives from starting
until
the dust collector
had reached complete operation, on or about March 22, 2007.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable
abatement method to
correct these hazards includes following the National Fire Protection
Association (NFPA) 484,
Standard for Combustible Metals, 2006 Edition, including, but not limited
to:
*Locate dry-type dust collectors outside of the building as listed in
Paragraph 12.2.1.4 of NFPA
484-2006.
*Provide dry-type dust collectors with barriers or other means for
protection of personnel as
listed in Paragraph 12.2.1.5 of NFPA 484-2006.
*Ensure that ductwork is as straight and short as possible with as few
bends and irregularities as
possible to prevent interference with free airflow as listed in Paragraph
12.2.5.5 of NFPA
484-2006.
*Construct ductwork of conductive material and assemble ductwork with
smooth interior
surfaces with internal lap joints facing in the direction of airflow, and
with seams oriented in a
direction away from personnel as listed in Paragraphs 12.2.5.6 and
12.2.5.8 of NFPA 484-
2006.
*Bond and ground duct systems, dust collectors, and dust-producing
machinery to minimize
accumulation of static electric charges as listed in Paragraph 12.2.5.10
of NFPA 484-2006.
*Interlock the power supply to the dust-producing equipment with the
airflow from the dust
collector, so that improper functioning of the dust collection system will
shut down the
equipment it serves as listed in Paragraph 12.2.8.1 of NFPA 484-2006.
*Provide a time delay switch or the equivalent on dust-producing equipment
to prevent the
starting of the motor drive on the equipment until the collector is in
complete
operation aslisted in Paragraph 12.2.8.2 of NFPA 484-2006.
*Remove dust from dry collectors at least once each day and at more
frequent intervals if
warranted as listed in Paragraph 12.2.10.6 of NFPA 484-2006.
*Provide dry-type dust collectors used for metal dusts with deflagration
vents as listed in
Paragraph 12.2.10.7 of NFPA 484-2006.
