Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to falls to lower levels:
(a)Foundry - Employees used a wooden, 12-foot long, 2x10 plank as a work
platform while
performing servicing and maintenance work on the rotary drum.  Employees
also used the
wooden plank on a daily basis when readying the rotary drum for
production.  At one end, the
wooden plank rested on an 11-foot high, fixed, metal platform at the
entrance or hopper end of
the rotary drum.  At the other end, the wooden plank rested on a 10-foot
high, fixed, metal
platform at the exit end of the rotary drum.  Nothing secured the wooden
plank from
movement nor did the plank sufficiently overlap either of the fixed, metal
platforms upon
which its ends rested.  On July 12, 2008, as two employees stood near the
end of the wooden
plank which rested on the 11-foot high metal platform at the hopper end of
the rotary drum,
that end of the wooden plank fell, causing the employees to fall to the
floor.  One of the
employees suffered fatal injuries in the fall.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable
abatement method to
correct this hazard includes securing the wooden platform from movement by
cleating or otherwise
restraining it by hooks or equivalent means at both ends or extending the
platform no less than six
inches and not more than 18 inches over the end supports.
