were exposed to a fire & explosion:
At this inspection site, on March 08, 2007, and at other times prior
thereto, the
auxiliary escape line was installed in an unsafe manner and included the
following
visible deficiencies:
a.A securely anchored one-half inch wire rope escape line was not attached
to derrick or mast so as to provide a ready and convenient means of
escape from the Derrickman's working platform.
b.The Emergency Escape Device was installed on the wire rope escape line
which was secured to a shackle that went through a hole in a pad-eye at
the top of fabricated piping above the walking surface of the monkey
board which permitted the upside down "T" bar to be below the walking
surface of the pipe racking board.
c.The one-half inch wire rope escape line was not stretched out and
securely
anchored at ground level so as to provide a ready and convenient means
of escape from the Derrickman's working platform.
d.The one-half inch wire rope escape line was approximately twenty feet
short and would not assure a safe landing and allow the Derrickman to
stop twenty to twenty-five feet from the anchor point.

At this inspection site, employees working at the Blowout Preventer/cellar
area
were using a three-inch diaphragm pump with a three-eighths-inch rubber
hose
containing compressed air and did not install keeper pins/wire in the ears
of the
crowfoot connection to prevent them from unintentional parting.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be to require that keeper pins/wire be installed in each
ear of
the crowfoot connection prior to use per the manufacturer's
recommendations.
