Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by a hydraulic vehicle lift: a) On or about March 30, 2007, the employer failed to ensure that the safety locking device on the Rotarty Automotive lift number two, was being utilized. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow ANSI/ALOIM-200, (Operator Responsibilities), Section 4.4.4, (Operator Reporting) Section 4.5, (Qualified Inspection Procedures), Section 5.1, and (Inspection Points), Section 5.62.16. THE RAMDEV CORPORATION WAS PREVIOUSLY CITED FOR A VIOLATION OF STANDARD OIL X-CHANGE OF GRAYSLAKE WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 310433636, CITATION NUMBER 1, ITEM NUMBER 1, ISSUED ON OCTOBER 13, 2006, WITH RESPECT TO A WORKPLACE LOCATED AT GRAYSLAKE, IL. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that the abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
