Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a burn/fall hazard:
At this inspection site, on April 11, 2007, and at other times prior
thereto, the
auxiliary escape line was installed in an unsafe manner and included the
following
deficiencies:
a.A securely anchored nine-sixteenths wire rope escape line was attached to
derrick or mast so as not to provide a ready and convenient means of
egress from the Derrickman's working platform.
b.The Emergency Escape Device was installed on the wire rope escape line
which was secured to the crown of the derrick and was attached to the top
guardrail of the monkey board by a chain that went over the wire rope
escape line and around the top guardrail and tied in a "Hog knot."
c.The Emergency Escape Device upright bar was away from the pipe
racking board with the upside down "T" bar at the walking surface of the
pipe racking board or below it.
d.The nine-sixteenths inch wire rope escape line was stretched out, with
too
much slack.
e.The end of the wire rope escape line had been placed in the grooved area
of a thimble to form an eye with the dead end of the wire rope extending
back beyond the thimble and laying along side of live end.  Only two
Crosby clips were used to secure the dead end of the wire rope to the live
end and they were installed backward, i.e. saddle on the dead end and the
"U" bolt on the live end.
f.The end of the wire rope escape line was improperly secured.  A chain
was passed through the thimble eye with a hook on the chain which was
hooked back into the chain after the rest of it passed around the stake and
then wrapped around the chain and then tied into a "Hog knot."
One feasible and acceptable abatement method would include ensuring the
Auxiliary Escape is installed according to manufacturer's recommendations.
