Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to anhydrous ammonia hazards: On or about May 13, 2007, employees were exposed to anhydrous ammonia when a pressure relief valve located next to the air intake of the boiler room activated. The released ammonia was pulled back into boiler room and the room adjacent to it. There was no fixed ammonia detection system in the boiler room. Among other methods, feasible means to correct these hazards include: 1. Follow the American National Standards Institute/American Society of Heating, Refrigerating and Air Conditioning Engineers, ANSI/ASHRAE 15-2004, Safety Standards for Refrigeration Systems, service and maintenance provisions as applicable. 2. Follow the International Refrigeration Bulletin #110, 3/93: Guidelines for Start-up, Inspection and Maintenance of Ammonia Mechanical Refrigeration Systems, inspection and maintenance provisions as applicable.
