recognized hazards that were causing or likely to cause death or serious
physical harm to
employees in that employees were exposed to crushing injuries in the event
of a forklift tip-over
or other accident.
a) On or about May 16, 2007 in the lumber receiving and storage areas -
The operator
of a Taylor TYB 180 M forklift was not wearing a seat belt while unloading
and stacking
rough cut lumber.
Feasible means of abatement include; observe powered industrial truck
operations to
determine if trucks are being operated safely, and conduct
employer/employee interviews
to verify training program implementation.
