Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Warehouse: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees, in that employees were exposed to the hazard of rollover of equipment or sudden impact with a fixed object resulting from the employee being ejected from the operators station: (a) On May 23, 2007, the employee driving lift #7 was not wearing the seat belt provided on the lift. (b) On June 4, 2007, the employee driving lift #15 was not wearing the seat belt that was provided on the lift. ABATEMENT CERTIFICATION AND DOCUMENTATION NOT REQUIRED
