Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burns/crushing: a) Press Room - Press # 23: Employees exposed to hand crushing and burn injuries when the hydraulic injection molding press unexpectedly failed causing massive steel plate to fall to bed of press. Violation disclossed on or about 5/7/2007 and 5/15/2007. Among other methods, one feasible and acceptable method of abatement to correct this hazard is to: 1) Ensure machine operators are trained to utilize wood blocks during mold replacement and de- molding process per manufacturer's recommendation in the Lewis Welding & Engineering Corp. Rubber Injection Molding Machine Operator's Manual, Section 4.3. 2) Ensure maintenance personnel are trained and perform preventative maintenance, including lubrication, conducted in accordance with the manufacturer's recommendation in the Lewis Welding & Engineering Corp. Rubber Injection Molding Machine Operator's Manual, Section 4.5.
