P.L. 91-596 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard(s) of an acetylene fueled deflagration and/or fire: a)Near the Northwest corner of the production building, on or about April 15, 2009, employees were working around a pressure vessel containing highly pressurized acetylene that leaked at the main flange, releasing the gas into the general work area where numerous ignition sources existed. Among other methods, feasible and acceptable means of abatement are 1) to tighten the flange bolts in accordance with ASME PCC-1-2000, Guidelines for pressure boundary bolted flange joint assembly. ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.TION
