crushed by the industrial lift truck:
A) Bldg. 5087, on or about November 10, 2009, an employee was operating a
Hyster 45 forklift
without wearing the seat belt.
One feasible and acceptable abatement method to correct this hazard is to
establish and enforce
a written policy requiring industrial forklift truck operators to wear
seat belts.
ABATEMENT CERTIFICATION IS NOT REQUIRED.
