employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm:
Employee(s) was exposed to the hazards of oil and gas well blowouts, fire,
and/or derrick
collapse. Employee(s) working at various levels of a well service rig mast
was provided only
one means of egress, the mast ladder, the use of which could be blocked by
the preceding
hazards.  A secondary, emergency only, means of egress to remove the
employee(s) from the
proximity of the hazards could limit their exposure.  Among other methods
to provide a
secondary means of egress, one feasible and acceptable means would be the
installation and
expeditious use of a properly rigged and secured "Geronimo" system.  An
employee was
working on rig mast mounted rod board on or about May 4, 2007, during the
removal of tubing
from an oil/gas well by the employer's Rig #210 at the Dark Star #2 lease,
Purcell,
Oklahoma.
