Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: On or about October 29th, 2007 the employer did not ensure the LPG heater was provided with adequate ventilation before use. Employees potentially exposed to burns and smoke inhalation due to fire.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: On or about October 29th, 2007 the employer did not ensure employees were adequately trained to use equipment, such as the propane heater in the dog house. The heater was installed and used in an enclosed trailer with inadequate ventilation.
