On or about March 13th, 2008 the employer did not ensure the wire rope
(line) for the
emergency escape device, for the employee working in the derrick, was
attached at the ground
to an anchor point.  Among other methods, one feasible and acceptable
abatement method to
correct this hazard would be to ensure the line for the emergency escape
device is anchored
before starting any servicing job requiring an employee to work on the
tubing board or any other
part of the mast.
