employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm:
On or about March 13th, 2008, the employer did not ensure the wire rope on
the air hoist was
properly rigged with an appropriate splice.  The employer had two clamps
with one of the
clamp's saddle mounted in reverse.  Also, the employer attempted to splice
the wire rope back
into it's self to create an eye.  The laying of the strands did not comply
with the manufacturers
recommendations for properly splicing wire rope.
