Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to being struck by the mast in the event of collapse. For the employees operating the service rig on or about May 28th, 2008. The employer did not ensure guy wires to the mast were properly rigged to the anchor points. A feasible method of correction is to properly install "U" Bolt style clamps in accordance with manufacturer's guidelines and recognized rigging practices (Rigger's Handbook).

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to explosion or violent release of fluid and or gas from a high pressure system connected to a gas well. a) For the employees working on the service rig drilling floor on or about May 28th, 2008: The employer did not ensure a person was on the floor qualified in well control procedures. b) For the employees working on the service rig on or about May 28th, 2008: The employer did not ensure personnel designated to operate the Blowout Preventer System understood the system and were able to safely operate the system. Among other methods of feasible abatement, the employer should have a person competent in well control procedures on the rig floor and train personnel on the rig to understand the operation of the blowout preventer system.er
