likely to cause death
or serious physical harm to employees:
The employer failed to ensure that the place of employment was free from
recognized hazards
in that:
a.At Durango Medical Plaza construction project, using unapproved lifting
equipment to erect structural steel, an employee was fatally injured when
he was
struck by the steel I-beam that fell from the jib and beam clamp
attachment on the
rough terrain forklift, Ingersoll-Rand, Model VR-90B.  Iron workers were
using
the forklift for lifting steel I-beams into place, instead of following
the guidelines
in 29 CFR 1926, Subpart "R", approved hoisting methods.  The steel I-beam
was
39 feet long and weighted over 2100 Lbs.
One feasible and acceptable abatement method would be to follow
requirements set in 29
CFR 1926, Subpart R - Steel Erection.
