Nevada Revised Statute 618.375(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: The employer failed to ensure that the equipment operators manual and maintenance records were available to address any equipment safety matter: a.During the inspection, the Safety Inspector observed a gate rail on the JLG Scissor Lift Work Platform that was missing a chain. Reference: ANSI/SIA A92.6 1990 for Self Propelled Elevating Work Platforms, Page 17, paragraph 7.2 Manuals: Users shall keep and maintain a copy of the operating an maintenance manual as required. The operating manual shall be provided with each rental, lease or sale delivery and shall be stored in the weather resistant storage compartment. The manual is considered an integral part of the aerial platform and is vital to communicate necessary safety information to users and operators. b.Reference: ANSI/SIA A92.6 1990 for Self Propelled Elevating Work Platforms, Page 17, paragraph 7.3, Inspection and Maintenance: Users shall inspect and maintain the aerial platform as required to ensure proper operation. The frequency of inspection and maintenance shall be determined by the manufacturer's recommendations and be compatible with operating conditions and the severity of the operating environment. Aerial platforms that are not in proper operating condition shall be immediately removed from service until repaired. One feasible means of abatement is to ensure that the manual is available to the operator(s) and maintain all maintenance records.
