Location: On the south side of the Holiday Inn Express, entrance side of
the building, located

Condition: Employee was observed working from an elevated work platform
approximately 28
feet above the ground level on a Lull #412-06-8400 #644B-42 Model Rough
Terrain Forklift
Truck without a personal fall arrest system or guardrail system. The
employee was using the
Lull rough terrain forklift to ride the forks and mask to the access the
third floor in order to get
the welding machine.
Among other means, one feasible and acceptable abatement method would be
to
provide a
platform which complies with the design requirements listed in the
American Society of
Mechanical Engineers B56.6-1992 Section 5.15 Elevating Personnel.
The employee was instructed to get down and not perform this unsafe
operation again.
Abatement was completed on the job site the day of the inspection
6-15-2007.
No abatement certification or documentation is required for this item.
