Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck sharp pieces.
PVC pipe was being used to transport compressed air....
"Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to:"
Only use material with shatter resistance properties. (Reference Plastic
Pipe Institute document).
2. Follow manufacturer recommendation on application.
Abated by replacing the PVC piping with with special pneumatic hose
throughout the
maintenance facility.
Date: On or about Wednesday October 24, 2007
Location: Throughout the maintenance facility located at 940 E. Main
Street Lavonia, GA 30553
Equipment: 3/4 PVC piping
Condition: The employer failed to ensure that the compressed air was
distributed in shatter
resistant pipes.  Mr. Charles Sewell the Senior Mechanic indicated that he
had
been using the
permanent mounted PVC pipe since he has been working for Columbia Farms of
GA, Inc.  Mr.
Sewell indicated that he has been working at the site for approximately
nine years.  Mr. Sewell
also indicated that he uses the air from the PVC pipe often to perform
maintenance of Columbia
Farms of GA, Inc equipment.
AMONG OTHER METHODS, A FEASIBLE ABATEMENT METHOD TO CORRECT THIS
HAZARD WOULD BE BUT NOT LIMITED TO: The replacement of the PVC pipe with a
type
of pipe approved for use with compressed air e.g. ABS or other approved
piping.
No abatement certification or documentation is required for this item.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to: amputation while operating, repairing and cleaning the
separator
and tray
cleaning machines without site specific procedures and training on
lockout/ tagout.
Date: On or about Wednesday October 24, 2007
Location: Separator and Tray washing rooms located at 940 E. Main Street
Lavonia, GA 30553
Equipment: Separator model (SEP-A1-600) and Traywasher model (HPTW)
Condition: The employer failed to develop an adequate lockout/tagout
procedure for the hatchery
area.  The energy control procedures that the employer had implemented
were not site specific.
The procedures contained general information about lockout/tagout. There
was no information
about equipment, energy source/magnitude of the energy that the equipment
utilizes.  Also, the
employer failed to develop an adequate energy control training program.
After reviewing safety
inadequate lockout/tagout policy the employer did not identify all of the
elements required for
energy control training for the authorized, affected and operating
employees' of the equipment.
The employees indicated that they are required to perform
maintenance/repairs
on equipment
weekly.
AMONG OTHER METHODS, A FEASIBLE ABATEMENT METHOD TO CORRECT
THESE HAZARDS WOULD BE BUT NOT LIMITED TO: The employer developing sites
specify procedures for the requirements for removal of such devices,
transfer of locks/tags or
responsibility for them, and testing for determination of lockout/tagout
effectiveness.
Also, the employer shall establish, communicate, train in use, and enforce
procedures for
lockout/tagout of energy sources to all authorized and affected employees.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation may include, but is not limited
to, evidence of the
purchase or repair of the equipment, photographic or video evidence of
abatement, or other
written records.
