Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the struck by hazards when dismantling a Hydro Mobile Mast
tower section
Model# P2K3j P Series motorized unit:
(a) Location - 52 Oxford Street, 3rd floor, East Vent Shaft, Cambridge, MA:
On or about June 29, 2007, employees were exposed to struck by hazards
while the
employer did not ensure that the mast section was dismantled according to
the
manufactures recommended procedures associated with removing each mast
tower
section. As a result of removing the four (4) bolts to remove the mast it
was
Among other methods, feasible and acceptable means of abating this
condition includes
but not limited to:
1. Follow Mast Mobile Operator's Manual# P2K3J, P Series motorized unit,
page 24,
"Fasten all tower bolts securely before raising platform."
2. Utilize only qualified workers in the erection and dismantling process.
3. Competent person needs to insure that the piston arm is free from the
mast section to
be removed prior to removing the tower bolts.
4. Follow Mast Mobile Operator's Manual# P2K3J, P Series motorized unit,
page 24,
"For personal safety, the use of a sling is recommended when manipulating
such mast
sections.
5. Platform electrical controls should be modified so that the up and down
operation
switch position on the scaffolding platform is consistent with standard
switch
lever
positions.
