Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to being struck by a 36 inch concrete pavement saw blade:
a)Edgewater Road:  The Diamond Products concrete saw, Model CC7172, was
missing the 5/8 inch split lock washer.  The saw blade was not mounted
with the
blade flange bolt and lock washer, on or about 10/2/2007.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to:
1.Follow the manufacturers printed instructions which requires the use of
a 5/8 inch split
lock washer Part No. 2900113 when mounting the blade.
2.Train operators in accordance with the Diamond Products concrete saw
operator's
manual for Model CC7172.
3.Train operators to use the Diamond Products concrete saw parts list for
Model CC7172.
