Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of being crushed when working beneath
and/or
alongside a wheel loader:
a)Work yard behind shredder / crusher machine operator's house:  Employees
working on a Daewoo Mega 400-V wheel loader were exposed to the hazard
of crushing injuries or death, in that tree log support stands were used to
elevate this heavy equipment while employees worked underneath and/or
alongside, and that the front wheels were not blocked.  The stands
supporting
capacity was not determined and proof tested, on or about 4/10/2008.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable method to
correct this hazard is to:
1:Purchase and use commercially manufactured vehicle (automotive) support
stands that
incorporate the requirements of ASME Portable Automotive Lifting Devices
(PALD) - 1993,
section 4.1.2.
As described in ASME PALD - 1993, section 4.1.2, vehicle support stands
shall be capable
of performing the proof load test with a load of 150of rated capacity.
The ASME definition of "proof load" is a load, greater than the rated
capacity, applied
centrally to the lifting or attaching points of the PALD to confirm the
integrity of the
structure.
Additionally, as described in Section 1 of the Operation & Maintenance
Manual, provided
through Daewoo Heavy Industries & Machinery LTD., it states the use of
solid support
blocking and the use of adequate equipment supports and blocking.  Manual
states to block
wheels fore and aft to prevent any movement, and do not use hollow,
cracked or unsteady,
wobbling weight supports.
The ASME definition of "appropriate means" is the use of any device, such
as blocking,
cribbing or vehicle support stands, which adequately support a vehicle off
the ground by
providing a stable support preventing the vehicle from tipping over of
falling down to the
ground.
