Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to loads falling from hoist hooks: a)700 Schuyler Avenue, Lyndhurst, NJ: The hook of the 1 ton P&H hoist on the jib crane had a broken safety latch, on or about 01/08/2009. b)700 Schuyler Avenue, Lyndhurst, NJ: The hook on the 2 ton P&H hoist on the Robbins & Myers crane had a broken safety latch, on or about 01/08/2009. ABATEMENT NOTE: Among other methods, one feasible and acceptable method to correct this hazard is to equip the hoist hooks with safety latches as described in ANSI B30.16-1993 Section 16-1.2.9 (Hooks). A latch shall be provided to bridge the throat opening of the hook and retain, under slack conditions, such items as, but not limited to, slings and chains.
