were exposed to loads falling from hoist hooks:
a)700 Schuyler Avenue, Lyndhurst, NJ:  The hook of the 1 ton P&H hoist on
the
jib crane had a broken safety latch, on or about 01/08/2009.
b)700 Schuyler Avenue, Lyndhurst, NJ:  The hook on the 2 ton P&H hoist on
the
Robbins & Myers crane had a broken safety latch, on or about 01/08/2009.
ABATEMENT NOTE:  Among other methods, one feasible and acceptable method
to correct
this hazard is to equip the hoist hooks with safety latches as described
in ANSI B30.16-1993
Section 16-1.2.9 (Hooks).  A latch shall be provided to bridge the throat
opening of the hook
and retain, under slack conditions, such items as, but not limited to,
slings
and chains.
